Step 3: Implementing Select Foundational Programs & Capabilities

During Step 3: Implementing Select Foundational Programs & Capabilities, local health departments will form an implementation leadership group that will consider various delivery methods, engage partners if appropriate, and create implementation and evaluation plans that are communicated to public health staff and community partners.

 

Objectives
  • Health department staff, with subject matter expertise and defined roles and responsibilities, has reviewed current programs and processes in the priority area and has determined best methods to achieve the goals and meet metrics established in given available resources
  • Staff and partners have determined the best, most efficient service delivery method that fits the resources available (after consideration of various models including cross-sector partnerships and cross-jurisdictional sharing)
  • If they haven’t already, and if appropriate, partners from other counties or sectors have been engaged for collaborative service delivery
  • A written plan for implementing changes is created with an accompanying evaluation plan
  • All staff and appropriate partners understand the basics of the implementation plan, including the service delivery method
  • Implementation activities are carried out and monitored

Critical Strategies

An implementation group led by public health leadership and staff will be formed. The implementation group will include public health staff with subject matter expertise. Each group member will be given defined roles and responsibilities. The group will lead the work to achieve the goals and outcome metrics established in step 2.6. At the conclusion of this strategy, the implementation group will have reviewed current programs and processes in the priority area and determined the best methods to achieve the goals and meet the metrics that were establish.

Public health staff will work with partners to review possible delivery systems and models (such as cross-jurisdictional sharing, cross-sector sharing, contracting, in-house models, etc.) for the proposed implementation. Decisions around delivery model will be based on adding value or creating efficiencies by reviewing and considering different approaches. Staff and partners will use these reviews and tools to determine the best and most efficient service delivery method that fits the resources available.

During the Engage Partners in Service Delivery strategy local health departments will use provided tools to help guide decision-making related to the roles and responsibilities of each partner. At the conclusion of this critical strategy, each partner will have a clear description of their role in service delivery.

During the Create Program or Capability Specific Plan strategy, public health department staff and leadership will determine what, specifically, will change, how these changes will lead to the accomplishment of goals/achievement of outcomes, when these changes will take place, and who will execute these changes. At the conclusion of this critical strategy a written plan for implementing these changes and an accompanying evaluation plan will have been created.

During the Communicate Plans with Staff and Partners strategy, both partners and local health department staff should continually be informed of updates and changes to the implementation plan. Once an implementation plan and evaluation plan have been considered and created, all staff and partners will be informed. This information will result in all staff and partners understanding the basics of the implementation plan, as well as the service delivery methods. Keeping all partners informed of updates and changes to the implementation plan and service delivery model will ensure partner buy-in, consistency, and best practices throughout the implementation process.

The Execute Changes strategy is to execute all implementation activities. These activities will be continually monitored as the implementation is carried out so that in future steps and strategies the implementation plan and program can be tweaked as needed.
