Step 4: Evaluate Process and Progress and Make Necessary Adjustments

During Step 4: Evaluate Process and Progress and Make Necessary Adjustments, local health departments will use the evaluation plan to determine regular evaluation meetings, make mid-course adjustments, and track progress towards outcomes. Continual staff and partner reports will be developed that detail the implementation progress, successes, and lessons learned.

 

Objectives
  • PH has established regular time intervals for monitoring implementation (monthly, quarterly, etc.)
  • Process, output and outcome evaluations occur as planned.
  • Midcourse adjustments are developed and operationalized
  • Ongoing evaluation activities are in place
  • Report to staff, decision makers. Share successes and lessons learned

 

 

Critical Strategies

During the Assess Implementation Status and Identify Challenges and Successes strategy, local health department staff and the implementation group will determine time intervals for assessment and for convening partner status meetings throughout the implementation process. These assessments are reviewed to identify challenges and successes to implementation as well as the status of implementation.

During the Make Mid-Course Adjustments strategy, the implementation group, public health department staff, and partners will use the assessments and status meetings from critical strategy 4.1 to work together to make mid-course adjustments where needed.

Tracking progress toward metrics and report outcomes is ongoing throughout the implementation process, and continuing after midcourse adjustments are made. Public health department staff will continually assess and evaluate the implementation of the program. These assessments will be used to improve the implementation process and will be reported to staff and decision makers.
